51Near Miss Electrocution

Brief Description of Incident/Accident & Description of Consequences/Outcomes:

A contract fitter engaged in the repair of a dragline lower roller path entered the tub 22KV cable connection chamber with the power still connected. He had found the chamber door open and was advised by his safety representative that the area was covered by an access permit for work on the roller path. He completed the job and left the chamber without being injured.

Visual inspections of safety locks and other such devices should form part of regular maintenance procedures.

The poor practice of leaving the door interlock switch taped in the closed position without notification and appropriate precautions is unacceptable. This should be made clear to all electrical employees.

In planning maintenance work on draglines and other large machinery, areas to be accessed should be clearly identified, together with their points of isolation. A plan showing the various parts of draglines and other large machinery and indicating points of isolation of all energy sources to access these parts should be drawn up and used to prepare access permits.

After isolation all personnel involved in the work should be made aware of the areas to which they have access. Where necessary, other areas should be barricaded off.
