2Compressed Air Pipeline Failure

Brief Description of Incident/Accident & Description of Consequences/Outcomes:

A length of galvanised pipe used for carrying compressed air into an underground mine failed when a section of the pipe wall failed under pressure.

Identified Root Causes:

The investigation has revealed that the pipe range had been installed an indeterminate number of years ago and had been in constant use over that period to supply compressed air to the underground workings. The section of pipe that failed has indicated that the pipe walls are approximately "paper thin" and could not withstand the compressed air pressure at this point.
