An articulated water truck was engaged to provide water for road construction through a creek crossing. The truck was required to traverse a 1 in 5 exit and entry ramp to the creek crossing and it had successfully negotiated the crossing on 1 occasion. On the second occasion it was traveling at approx. 2-5 kph when it rolled onto its side. Fortunately the operator suffered only slight injuries.


The incident occurred because the configuration of the vehicle not suited to the task, the fifth wheel assembly was not restrained, allowing the assembly to misalign by up to 90 degrees, the shifting load acted on misaligned fifth wheel assembly, and a pre-start inspection regime was not developed or utilised for the specific vehicle type.


Recommendations include ensuring that the equipment and vehicles engaged in activities are suited to the task, developing pre-start checklists and utilising them for vehicles on site, ensuring that the correct configuration of turntable variables is used for the task at hand when utilising articulated vehicles, conducting routine checking of the baffle configuration and integrity where shifting loads e.g. liquids are transported in tankers, and complying with the requirements of Section 74 of the Coal Mining Safety and Health Regulation and Section 100 1 (b) (iii) of the Mining and Quarrying Safety and Health Regulation relating to ROPS and FOPS.
