What

Crane operators have inadvertently released the auxiliary hoist, sending the hook (100kg) into an uncontrolled free fall.

Why

On both occasions the driver of the crane has inadvertently released the auxiliary hoist, resulting in the auxiliary hook free falling.

Recommendations

Recommendations relate to inspecting the crane before use to see if it has a free fall feature, if so make sure the controls of the feature are interlocked in a positive and fail safe manner, appropriate risk assessment is conducted when using the feature and adhering to the current Australian standards.
