What

The body of a Hitachi EH4500 rear dump truck contacted an overhead crane causing it to dislodge and fall to the floor.

Why

Contributing factors relate to the position of the crane, failure to check overhead, lack of hazard awareness, absence of documented procedures, inadequate workspace.

Recommendations

Develop documented procedure using risk assessment, install signage, review all cranes on site for similar potential and establish operating procedures including designated park-up area, investigate proximity detectors and alarms on cranes, include checking for overhead hazards on existing service sheets.
