Information:

What

While magnetite was being dumped from a G-524 "Kanga Loader", the operator leant forward to look into the pit, causing the loader to overbalance and catapult the operator over the top of the machine.

Why

The machine was handling a load greater than the recommended safe weight limit; the operator was not trained in the operating limitations for working with magnetite; there were no operator restraints fitted; no warning systems.

Recommendations

Control measures should address the selection of appropriate plant for particular activities. These should include: operating procedures for dealing with different materials; adequate training for operators; the safety of operators using machinery not fitted with appropriate safety restraints and roll-over protection.
